4 things you need for the Health Improvement Team (HIT) Call on Oct. 15 from 11:00 am - 12:00 pm

The October HIT Call is here. Here are four things you must know in preparation for the call.

1.  Topic:  

• WWC Clinical Services & Care Coordination Progress Report 
• Identifying WWC – Eligible Women – evaluation results & implications

2.  Purpose:  

The purpose of this HIT call is to discuss 1) the new progress report every WWC clinical services and care coordination grant program agency is required to complete via Survey Monkey, and 2) the findings of a recent evaluation project to learn more about WWC eligible women.

3.  Please review these documents before the HIT Call:

• Progress report worksheet. This worksheet will allow you to gather your agency responses and submit as one voice. 
• Test URL link to the progress report. Please note: you will be receiving the true URL to the progress report by Oct. 31
• Power point presentation on identifying WWC eligible women
• Discussion questions 

4.  Call-in Information:
