Provide advice: How do you talk to clients about care coordination?

The most frequently asked question I get about care coordination is, “How do I get insured clients to sign the consent form for care coordination?" It’s much easier to explain a program that pays for breast and cervical cancer screenings (WWC clinical services) than it is to explain a program that provides patient navigation services (WWC care coordination).

I know some of you have had some luck enrolling clients into WWC care coordination. What works for you?

  1. Great comments on the CCGP call today. Below are some suggestions from WWC agency staff on how to explain CCGP services.

    "We're here to provide individualized assistance to you throughout the process from getting your initial breast and cervical cancer screenings to diagnostic testing and treatment if needed."

    "Medicaid covers the screenings and diagnostic testing needed, but this program covers the patient navigation to assist in whatever barriers you may run into along the way."

  3. Please feel free to share how your agency is handling retroactive reimbursement from Medicaid when clients are initially enrolled in WWC clinical services.
