Two easy steps to enrolling a client in the Care Coordination Grant Program

Your agency has now gone through or is in the middle of the contracting process with CDPHE for the Care Coordination Grant Program (CCGP). That is the most difficult part. Enrolling clients into CCGP and getting paid for things you are already doing, well that’s a no-brainer on your part. Follow my quick, two step process to enrolling a client into CCGP:

  1. Check client’s eligibility
    • Is the client a woman?
    • Is she between the ages of 40 and 64 years?
    • Does she have proof of lawful presence in the United States?
    • Is her family income at or below 250 percent of the Federal Poverty Level?
    • Does she have insurance or is she eligible for insurance? 
  2. If you answered yes to all the questions above, have the client sign the Lawful Presence Affidavit (if not already on file) and the WWC/CCGP Consent Form (if not already on file).

  1. Our documented client eligibility screening would be the Client Profile Tool, correct?

  2. Hi Ellen- You are correct! The Client Profile Tool is an optional form, but all information on the Client Profile Tool must be collected somewhere in the chart (i.e. EHR).
