Help us welcome 10 new care coordination agencies!

In October, the Women's Wellness Connection executed care coordination contracts with 10 new agencies, two of which are solely care coordination grantees.

  • Colorado Alliance for Health Equity and Practice (CAHEP)
  • Community Health Services - Pitkin
  • High Plains Community Health Center
  • Hue N. Vo, MD
  • Jefferson County Public Health (CCGP only - no clinical services)
  • Planned Parenthood of the Rocky Mountains
  • Salud Family Health Centers
  • St. Joseph Hospital
  • Sunrise Community Health Center
  • Valley View Hospital Association (CCGP only - no clinical services)

    Great to see so many new agencies joining the Care Coordination program. Denver Health started the CCGP program in February of this year, and we have started to see a rise in our CCGP numbers in the past 2-3 months.
